The ROLE
As a ROCK Pathway Support Counsellor, you will help strengthen continuity between Home-Based Intervention and Live-In Treatment programs. You will support transitions between levels of care, helping youth and families move smoothly through services while improving overall client flow and access to care. Working 37.5 hours a week you will act as a case manager to ensure that assigned cases for “high need” families requiring intensive support are provided expedient access within their home and/or community.
This is a newly created, full-time contract position with a duration of 24 months
In this role you will:
Collaborate with the caregiver/family, child/youth, supervisor and interdisciplinary team to develop a coordinated care plan with goals that are client-centered, specific, practical, and achievable.
Consult with an interdisciplinary team and other community service providers to ensure coordination and collaboration of treatment to best meet the client’s needs.
Provide services that are sensitive to the cultural, linguistic, and social diversity of families within the home and/or community.
Provide flexible, responsive, and accessible services that are delivered in home, schools, and in the local community.
Use a variety of therapeutic intervention strategies, including instruction, modeling, parent support, individual, family, and group counseling.
Facilitates or assists with the facilitation of parent coaching and/or children’s treatment groups.
Provide in-home assistance to families requiring behaviour management and emotion regulation strategies, risk assessment and safety planning, parent coaching, life skills, and links to community supports.
Act as both an advocate and liaison for families with community agencies, schools, law enforcement, the medical community, and other local service providers.
Provide opportunities for children and families to develop and maintain social relationships and community linkages.
Consult with other staff members, as well as significant professionals and programs in the community regarding the direction of intensive services and/or appropriate service referrals.
Assist in the preparation of monthly statistics and year-end reports.
Complete all administrative tasks, including inputting data into the ROCK client database in a timely fashion, as required for both client and organizational purposes.
Operationalize client safety through adhering to established policies and guidelines that ensure consistency in the delivery of quality and safe care, and by identifying and reporting any client safety incidents.
Commit to supporting client-centered care by adhering to ROCK’s principles of respect, collaboration, and service planning, as outlined in the agency’s strategic plan.
Abide by the specific requirements of Section 28 of the Occupational Health and Safety Act, as well as any further health and safety responsibilities outlined in the Agency’s policy and procedure documents.
Other duties may reasonably be assigned.
Who we’re looking for:
Education:
Graduate of a recognized Honours university program (Bachelor of Social Work preferred), community college program in the field of Child and Youth Work, or equivalent education relating to the specific population to be served.
Experience
Experienced in the field of children’s mental health.
Evidence of a theoretical framework which guides the candidate’s clinical work, is compatible with the Agency’s approach, and reflects an understanding of crisis theory.
It is considered a valuable asset if the applicant has practice or training in one or more of the following skill areas:
Providing group treatment OR facilitating parent events.
EFFT, DBT skills, and/or Crisis training (ASIST, etc.)
Skills:
Excellent communication and interpersonal skills.
Strong organizational, administrative, and case management skills.
Sound decision making and problem-solving skills.
A proven ability to work independently.
