POSITION PURPOSE:
The Outreach and Enrollment Coordinator/CHW provides insurance enrollment opportunities for residents who lack access to healthcare and health insurance. This position provides enrollment and application follow-up assistance, builds/strengthens enrollment collaborations with local partner organizations, maintains an ongoing presence in the community (i.e. attending key local events) and increases health insurance coverage and access to quality primary health, dental, and behavioral care for the community.
ESSENTIAL FUNCTIONS:
Provides unbiased information to consumers about health insurance, the new marketplaces, qualified health plans, Medicaid (Oregon Health Plan).
Provides assistance to consumers over the phone and in-person.
Assists patients in enrolling in and navigating the patient portal.
Provides assistance to patients regarding the 340B pharmacy program (i.e., IDP).
Provides healthcare system navigation services.
Facilitates enrollment of eligible health center patients and services area residents into affordable health insurance coverage through the Oregon One System.
Provides enrollment assistance (including but not limited to completing applications, gathering documentation, and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost, and free health insurance programs through Oregon One System.
Provides structured patient education on health coverage, engages in follow-up conversations, and offers renewal assistance for enrolled individuals.
Distributes outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
Develops, discovers, and attends events to promote coverage options and the mission and services of the organization.
Collaborates with provider teams, patients, Oregon Health Plan, and other state agencies to ensure proper enrollment and eligibility determination to facilitate proper care.
Networks with Communities United Resource Network (CURN) to maintain and obtain knowledge of community resources in order to make referrals accordingly to patients.
Supports outreach/enrollment assistance personnel as needed, complies with, and successfully completes all applicable federal and/or state consumer assistance training required for carrying out consumer assistance functions.
Attends and represents the organization at trainings and meetings as required.
Participates in ongoing conference calls, webinars, and other professional development.
Assists in development and implementation of Aviva Health outreach and enrollment initiatives.
Collaborates with quality manager as it pertains to Oregon Health Plan patient enrollment data to ensure measures are met. (i.e., Wolf Creek Job Corp).
Coordinates Independent Drug Program as assigned.
Periodically travel using the company or personal vehicle for events or trainings.
Follows all Aviva Health policies and procedures.
Performs other duties as assigned.
QUALIFICATIONS:
High school diploma or GED required; Associate degree or higher preferred.
Oregon CHW certification required or to be obtained within 6 months from date of hire.
Two years of experience in a busy office setting.
Medical insurance knowledge preferred.
Experience with Electronic Medical Records preferred.
Knowledge of Federally Qualified Health Centers and income-based sliding scales preferred.
Knowledge of billing practices preferred.
Excellent communication skills and the ability to effectively interact with people from various backgrounds in a compassionate and knowledgeable manner; Bilingual preferred.
WORKING CONDITIONS:
Must be able to perform the following physical requirements:
Remain in a stationary position frequently throughout the day while working on a computer.
Move or traverse occasionally throughout the day for other relevant duties.
Operate equipment frequently throughout the day: Computer and other standard office equipment.
Ability to communicate information and ideas clearly and accurately so others will understand; ability to interact with patients and staff clearly.
Able to move or transport up to 30 pounds to set up for events.
Work indoors in heat-controlled environment frequently throughout the day with periodic outdoor events and potential exposure to heat and cold.
OSHA Exposure Category – Category #2: The normal work routine involves no exposure to blood, body fluids, or tissues.
Work is in-person during normal business hours; periodic evening or weekend work for events.
May be required to travel for training and events using company or personal vehicle.
