The Ontario Telemedicine Network (O.T.N.) and Psychiatry Clinic R.P.N. is responsible for assessment and liaison with medical specialists and community psychiatrist providing consultation services to individuals in the community through the O.T.N. and other virtual platforms. The R.P.N. works closely with the O.T.N. Telemedicine Coordinator and clinic psychiatrists to increase capacity, organize and schedule appointments, establish O.T.N. and virtual psychiatry clinics, assist with equipment set up, and provide consultation with patients who are meeting with the specialists. The R.P.N. is responsible for nursing assessments as directed and required by the physician during the consultation and provides patients with information as follow up to their appointments, as appropriate and required. The R.P.N. also acts as a resource providing consultation and education to staff and clients as required. The R.P.N. also has a role in ongoing quality improvement to O.T.N. and psychiatry clinic operations and to the development of a nursing community of practice within C.M.H.A. H.K.P.R.

Key Responsibilities:
1. Client Services
Acts as an organizational point of contact for clinical telemedicine and is responsible for the delivery of high-quality telemedicine services within the organization.
Completes nursing assessments as required by the consulting physician, which may include, blood pressure, weight, health assessment and follow up as required.
Receives and facilitates O.T.N. and psychiatry referrals through email, faxes, telephone or walk-in contacts.
Contacts physician’s offices to obtain client information.
Schedules for appointments through scheduling services
Supports communication between patients and physicians, including supporting clients in their understanding of instructions, where appropriate.
Provides post-appointment client support in dealing with the immediate impact of the consultation and providing clarification around consultation outcomes, as well as follow up instructions for patients
Assists patients in completing referrals as directed by the physician or requested by the patient.
Maintains patient medical records in accordance with C.M.H.A. H.K.P.R.’s policies and procedures, and with applicable privacy legislation and policies/procedures.
Transcribes and follows doctor’s recommendations, including possible injections of medications.
Provides support to both internal and external programs on telemedicine processes and technology.
Completes general administrative tasks including emailing, faxing and photocopying patient information as required for smooth clinic operations.
In conjunction with the O.T.N. coordinator and clinic psychiatrists, acts as a liaison /point of contact for clinical activities, this can include but is not limited to updates for network, policies, contact information, including communication of this information internally, as necessary.
Performs a role in ongoing quality improvement to clinic operations and to the development of a nursing community of practice within C.M.H.A. H.K.P.R.
2. Documentation
Complete all required documentation and statistical activities for C.M.H.A. H.K.P.R.
Accurate and timely completion of all other administrative documentation including time sheets, expense claims, vacation and time-off requests, and training requests.
Accurate and timely completion of progress notes, as per agency policy, on client record, in conjunction with data collection processes of service delivery partners, Nursing Assessments, metabolic monitoring, Medication Reconciliation and Best Possible Medication History summaries.
Participate in completion of mental health nursing assessments
3. Other Activities
In conjunction with the O.T.N. Coordinator and clinic psychiatrists, liaises with hospitals and medical agencies to establish and maintain contacts to enhance the services provided to the members of the community requiring appointments and follow up through the clinic.
Acts as an organizational point of contact for clinical telemedicine.
In conjunction with the O.T.N. Coordinator, and clinic psychiatrists participates in community outreach activities through the development of collaborative partnerships and through presentations, education and visits to community agencies and services.
With direction from the Program Manager, participates as an active member of the O.T.N. community of practice, attending regular meetings, disseminating information back to the O.T.N. team.
On an occasional rotational basis, fill in for other Nurses in other C.M.H.A. H.K.P.R. programs, during periods of absence, to ensure service continuation.
4. Performance Management and Professional Development
Participate cooperatively in the agency performance management process/system.
Stay current on relevant legislation and best practices, current research, policies and procedures.
Participate in ongoing professional development and mandatory training.
Actively engage and participate in supervisory process.
Maintains current certificate of competence as an R.P.N. with the College of Nurses of Ontario (CNO) and accepts responsibility for professional competency as a nurse in accordance with CNO’s practice standards.
Maintain membership in good standing with R.P.N.AO and have liability insurance.
Achieves and maintains active O.T.N. certification
5. Health and Safety – Worker Responsibilities
Work in compliance with the Occupational Health and Safety Act, applicable regulations and all organizational health and safety requirements and procedures to ensure the health and safety of clients, staff/colleagues, volunteers and students.
6. Client Safety
Adhere to C.M.H.A. H.K.P.R. client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.
7. Other Duties as Assigned
Attends, participates in team/staff meetings, and all-staff meetings as required.
Participates in Agency quality improvement activities, e.g. accreditation projects, committee work, special events, etc.
Participates in quality improvement activities at the team/program level.
Participates in external committees as required.
Duties may change from time to time.
Requirements of the Position:
A. Education
Successful completion of an approved Nursing Degree (R.P.N./R.N.).
Maintains current certificate of competence as an R.P.N. with the College of Nurses of Ontario (CNO) and accepts responsibility for professional competency as a nurse in accordance with CNO’s practice standards.
Maintain membership in good standing with R.P.N.AO and have liability insurance.

Assets with regards to this position
Successful completion of a university degree or college diploma in social/health sciences or related disciplines is an asset.
Additional Nursing certifications, such as Mental Health Nursing.
Formal O.T.N. Training.
B. Related and Relevant Experience
Mandatory Experience
3 or more years of related, preferably clinical, experience as a Registered Nurse/Registered Practical Nurse in a hospital or community setting.
Demonstrated knowledge of the Ontario Mental Health Act, The Health Care Consent Act, social service benefits, related legislation in Ontario and community resources in the Central East Local Health Integration Network.
Demonstrated health assessment skills, sound clinical judgment and decision-making skills.
Demonstrated knowledge of mental illness, concurrent disorders and crisis intervention.
Demonstrated ability, commitment to and knowledge of community health care.
Demonstrated ability to work collaboratively in an interdisciplinary team environment.
Demonstrated interpersonal skills used to work with clients in a diverse practice population.
Demonstrated ability to work independently, as well as, within a team.
Experience with and knowledge of medication injections including administration, documentation, monitoring, and side effects.
Demonstrated knowledge of computer applications (Outlook, M.S. Office, databases).
Preferred Experience
Experience in Nursing related to the mental health field, including working with addictions, dual diagnosis and/or challenging behaviours.
Experience in Community Nursing.
General nursing experience including cardiology and neurology.
Experience/knowledge of teleconferencing, video medicine, (particularly O.T.N.) equipment and scheduling practices.
Intermediate to advanced minus proficiency in French.

Working Conditions:
Works primarily in an office/clinic setting with occasional community appointments, including meetings in clients’ homes.
Regularly works in an office setting (C.M.H.A. H.K.P.R.- administrative offices) and in satellite locations, this role includes considerable travel time between work locations.
Works directly with individuals living with serious mental illness and/or dual diagnosis and/or addictions, individuals who are experiencing emotional and/or mental health issues and are working towards recovery.
Works directly with families of individuals living with serious mental illness, or who are experiencing symptoms of early psychosis.
Flexible hours to meet needs of clients at clinic.
Transports clients in personal vehicle.
There are multiple competing demands which require time management skills, the ability to be flexible and to prioritize.
Work in a remote environment, such as a personal home, as required.
