La Vita Bella Home Health and Hospice is seeking a compassionate and dedicated Occupational Therapist to provide high-quality, patient-centered care to individuals in the comfort of their homes.
In this role, you will evaluate patients’ functional abilities and develop individualized treatment plans to improve independence in daily living activities. You will provide skilled therapeutic interventions focused on self-care, home safety, upper extremity function, cognition, and adaptive strategies. Working collaboratively with physicians and an interdisciplinary care team, you will support patients in achieving greater safety, independence, and quality of life within their home environment. You will also educate patients and their families on adaptive equipment, energy conservation, and techniques to promote safe and meaningful participation in daily activities.

Responsibilities
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive living within the patient's capabilities.
Periodically participates with all other home care personnel in patient care planning.
Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
Writes reports to physicians regarding patient's progress.
Instructs patients/family members in home programs and fine motor movement exercises.
An initial evaluation, including plan of care and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary, if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. A summary is written upon patient's discharge.
Assists the physician in evaluating level of function.
Prepares clinical and progress notes.
Participates in QAPI activities as assigned.
Qualifications
Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA).
Currently licensed in the state of New Mexico.
Two years of experience (preferred).
Lifting Requirements
Ability to participate in physical activity.
Ability to work for extended period of time while standing and being involved in physical activity.
Moderate lifting.
Ability to do extensive bending, lifting, and standing on a regular basis.
