La Vita Bella Home Health is seeking a compassionate and motivated Occupational Therapist to join our growing home health team. We are dedicated to providing high-quality, patient-centered care that promotes independence, safety, and meaningful daily living in the comfort of our patients’ homes.

Position Type

Part-Time / PRN (Flexible Scheduling Available)

Responsibilities

Perform comprehensive occupational therapy evaluations in the home setting

Develop, implement, and update individualized treatment plans per physician orders

Address activities of daily living (ADLs), instrumental activities of daily living (IADLs), and functional mobility

Provide training in adaptive equipment, energy conservation, and home safety

Educate patients and caregivers on strategies to improve independence and safety

Accurately document evaluations, visits, and progress in compliance with CMS and agency standards

Collaborate with physicians, nurses, and the interdisciplinary care team

Maintain compliance with all federal, state, and accreditation requirements

Qualifications

Current Occupational Therapist license in the state of practice

Graduate of an accredited Occupational Therapy program

Home health experience preferred (not required)

Strong clinical judgment, organization, and communication skills

Ability to work independently in a home-based environment

Reliable transportation and valid driver’s license
