Campbelltown to Liverpool | NDIS | In-Home & Site-Based | 1 WFH Day
Sydney Health & Care Services (SHCS) is seeking an experienced Occupational Therapist to join our Allied Health team in a hybrid role, delivering services across community, in-home, and site-based settings throughout the Campbelltown to Liverpool region, with one work-from-home day per week.
This role suits an OT who values flexibility, autonomy, and strong clinical support while working closely with clients in their own environments.
About the Role
You will provide occupational therapy services primarily within clients’ homes, alongside sessions delivered at various SHCS sites. The role includes a hybrid structure, allowing for one designated work-from-home day each week for report writing, administration, and care planning.
You will manage a diverse caseload and work collaboratively with participants, families, and stakeholders to support functional independence and meaningful outcomes.
Key Responsibilities
Deliver OT assessments and interventions in clients’ homes and community settings
Attend and work across multiple SHCS sites as required
Complete functional assessments, home safety reviews, and capacity-building interventions
Prepare high-quality NDIS reports (FCA, AT, home modifications where relevant)
Use WFH time for documentation, reporting, and clinical planning
Collaborate with families, support coordinators, and allied health professionals
Maintain accurate, timely documentation aligned with NDIS requirements
About You
You are a confident, experienced OT who is comfortable working across locations and managing your time effectively in a hybrid environment.
You will bring:
Minimum 12 months post-graduate OT experience (NDIS experience preferred)
Strong clinical reasoning and professional report writing skills
Confidence working independently across home, site, and remote settings
Excellent communication and time-management skills
Essential Requirements
Bachelor or Master of Occupational Therapy
Current AHPRA registration
NDIS Worker Screening Check (or willingness to obtain)
NSW Driver’s Licence and access to a reliable vehicle
Working with Children Check
First Aid and CPR
