Haven is seeking a compassionate and skilled Occupational Therapist to join our home health team in the Broward County area In this role, you will serve as a partner in your patients’ journey to recovery—restoring mobility, promoting independence, and ensuring safety within the comfort of their homes.

You will help maximize each patient’s quality of life through personalized treatment, preventative care, and rehabilitative strategies, all under the direction of a physician-approved Plan of Care. Your expertise and encouragement will help clients regain their optimal level of function and improve their daily living.

Key Responsibilities:

Deliver skilled occupational therapy in accordance with the physician’s orders and individualized plan of care

Complete initial assessments and develop measurable, goal-driven treatment plans

Educate patients and caregivers on activities of daily living, home safety, and therapeutic exercises

Participate in interdisciplinary care planning with healthcare team members

Complete accurate and timely clinical documentation, including OASIS assessments

Monitor progress and adjust interventions as needed in collaboration with physicians

Attend patient care conferences and contribute to the overall care strategy

Ensure all documentation meets state, federal, and agency standards

Qualifications:

Current Occupational Therapist license in the state of Florida

Minimum of one year of clinical experience (home health experience preferred)

Strong understanding of rehabilitative and therapeutic practices

Proficiency in documentation systems (OASIS and EMR experience preferred)

Excellent communication and interpersonal skills

Ability to work independently and manage time effectively

Commitment to patient-centered care and professional growth
