Role Summary
The Nurse Practitioner (NP) will provide clinical oversight and patient care through a telehealth-based remote patient monitoring program. This role focuses on reviewing patient vitals, identifying trends, intervening early, and coordinating care in collaboration with physicians, care coordinators, and clinical support teams.
This is a hybrid remote position ideal for a clinically confident NP who values autonomy, proactive care models, and technology-enabled healthcare delivery.
Role Details
Conduct Virtual New Patient visits to qualify patients for RPM/CCM services. Place order for RPM and CCM services as deemed appropriate.
Conduct annual visits to update patient's file as appropriate. Review Pertinent Medical History and Medications during visits.
Provide support to patient care coordinators with complex patients.
Be able to coordinate and connect with multiple patients in a timely manner.
Timely chart completion expected. Volume is roughly 20-25pts daily.
Key Responsibilities
Review and interpret remote patient monitoring data (e.g., blood pressure, heart rate, glucose levels, weight, oxygen saturation)
Identify trends, abnormalities, and early warning signs requiring clinical intervention
Conduct telehealth visits and patient outreach as needed to assess symptoms, provide education, and adjust care plans
Collaborate with physicians and interdisciplinary care teams to escalate concerns and coordinate next steps
Document clinical assessments, interventions, and communications accurately and timely within the EHR
Support chronic condition management and preventative care initiatives
Ensure compliance with state scope-of-practice laws, telehealth regulations, and clinical protocols
Contribute to the continuous improvement of clinical workflows and patient monitoring processes
Qualifications
Required:
Active, unrestricted Nurse Practitioner (NP) license in applicable states
National NP certification (ANCC or AANP)
Minimum 1 year of clinical experience
Comfort practicing independently within defined clinical protocols
Strong clinical judgment and critical thinking skills
Proficiency with electronic health records and telehealth platforms
Preferred:
Experience with remote patient monitoring (RPM), telehealth, or virtual care
Background in chronic care management (hypertension, diabetes, cardiac conditions, etc.)
Multi-state licensure
Tech-forward mindset with adaptability to new tools and workflows
