Accountable to the Director, Integrated Health Systems, the Nurse Practitioner will demonstrate a high level of autonomy and expert skill in formulating clinical decisions when supporting residents within central west long term care (LTC) facilities, and potentially retirement homes, congregate and other settings in the community as part of the integrated health portfolio. The Nurse Practitioner will work to full scope of practice, utilizing all nurse practitioner core competencies (client care, quality improvement and research, leadership and education).

Accountabilities:

• Support multiple LTC homes providing time sensitive acute/episodic, palliative, geriatric, and repatriation visits to LTC residents in order to avoid unnecessary Emergency Department transfers, shorten length of stay and support sustainable discharges
• Build capacity of LTC staff through education and innovation as well as improving system integration through access to care, seamless transitions and partnerships
• Play a leadership role by performing quality improvement, applying/performing research and best practices, participating in innovative approaches to care to improve quality and safety in LTC
• Develop strong relationships with various community partners and throughout Osler and may act as a resource and consultant to individuals and groups within the professional community and other hospitals/agencies
• Collaborate with stakeholders to facilitate change and promote staff and patient/family satisfaction within a philosophy of patient and resident-centered care

• Master’s degree in nursing required
• Current RN (EC) registration with the Ontario College of Nurses required
• BCLS required
• Recent and relevant experience practicing as a nurse practitioner in geriatric, palliative, emergency or primary care required
• Demonstrated experience with nursing staff, patient and family health teaching using adult educational principles and evidence-based practice preferred
• Demonstrated excellent interpersonal, communication, problem-solving, conflict-resolution and decision- making skills required
• Ability to autonomously order and interpret tests, formulate a diagnosis, develop and communicate a treatment plan for complex seniors in LTC and community settings required
• Ability to communicate and counsel effectively related to goals of care required
• Proficiency in computer skills and data management required (e.g. Microsoft Excel and Word, Meditech, Connecting Ontario) required
• Required to work in and travel between all Osler sites, LTC homes and other community settings as required (valid Ontario drivers license and access to a vehicle is required)
• Fluent in English (comprehension, reading, writing, and speaking)
• Ability to act as a change agent
• Vulnerable sector check or attestation as it pertains to the Fixing Long Term Care Act required

• Demonstrated knowledge of resident, patient and family-centered care
• Demonstrated experience with resident and family health counselling
• Exemplary clinical assessment skills, knowledge and expertise
• Experience in and solid understanding of the intersections between acute care, long term care, and community care as it relates to repatriation and resources preferred
• Demonstrated ability to work both autonomously and collaboratively as a member of an inter-professional team and prioritize resident needs required
• Excellent organization, communication, interpersonal skills, problem solving and critical thinking skills required
• Experience performing assessments related to geriatric syndromes, palliative care, COVID-19, influenza, pneumonia, urinary tract infections, wounds and falls required
• Demonstrated ability to effectively address sensitive issues with families and staff
• Demonstrated leadership, quality improvement and research skills preferred
• Demonstrated commitment to continuous learning, professional development and reflective practice as it applies to the profession and related field
• Demonstrated ability to work in a stressful environment
• Knowledge and adherence to IPAC measures and directives required
• Providing care in a compassionate and caring manner within a diverse cultural environment
• Knowledge and understanding of the Occupational Health and Safety Act

• Excellent attendance, punctuality and work record

Hours: Currently days including the potential for weekends, evenings and statutory holidays based on program needs (subject to change in accordance with operational requirements)

Minimum: $64.02 per hour

Maximum: $79.74 per hour
