We are seeking a dedicated MDS Nurse who will also assist with floor nursing duties to support our residents and interdisciplinary team.
✨ Position Overview
This dual-role position is responsible for coordinating and completing MDS assessments, ensuring regulatory compliance, and supporting direct resident care on the floor as needed. Ideal for a nurse who enjoys both clinical documentation and hands-on care.
📋 Key Responsibilities
MDS / Clinical Documentation Duties:
Complete and coordinate MDS assessments (OBRA/PPS as applicable)
Ensure accuracy and timeliness of all assessments in PointClickCare (PCC)
Participate in care plan meetings and interdisciplinary team (IDT) discussions
Monitor documentation to ensure compliance with CMS and KDADS regulations
Assist in development and updates of individualized care plans
Track due dates and maintain MDS schedule
Floor Nurse Responsibilities:
Provide direct nursing care including medication administration and treatments
Monitor and document changes in condition, behaviors, and health status
Communicate effectively with physicians, DON, and IDT members
Respond to behavioral health needs with professionalism and therapeutic approach
Ensure a safe and supportive environment for residents
✅ Qualifications
Current Kansas RN or LPN license (required)
Experience with MDS 3.0 preferred (willing to train the right candidate)
Experience in long-term care and/or mental health strongly preferred
Knowledge of Medicaid regulations and documentation standards
Strong organizational skills and attention to detail
Ability to work both independently and as part of a team
