Available shifts:

Tuesday - Saturday 9am - 5pm w/ flexibility.

GENERAL SUMMARY: (General Purpose of the Job)

Habilitation/Companion/Transportation; Performs duties as delegated by Family Support Coordinator(s) in the community for persons with intellectual disabilities and/or behavioral health. Work with individuals on their individualized service plan (ISP) outcomes in their home or in the community.

ESSENTIAL FUNCTIONS: (Primary Responsibilities)

• To participate in the ongoing communication regarding program planning and implementation for individuals.

• Provide a safe, clean, comfortable and therapeutic environment during activities in the community.

• Encourage, guide and train individuals in developing daily living skills and independence skills in accordance with their ISP.

• Assist in organizing and participating in leisure/recreation activities.
