Band 6 Occupational Therapist – Neuro Early Supported DischargeLocation: Southampton (Hybrid – community and home visits)Hours: 8:00 – 16:00, Monday – Friday (some flexibility)

About the RoleWe are seeking a Band 6 Occupational Therapist to join the Neuro Early Supported Discharge Team. This hybrid role involves supporting patients with neurological conditions in the community, combining home visits with office-based administrative work.

Key Responsibilities

•
Conduct assessments and deliver occupational therapy interventions for patients with neurological conditions

•
Travel to patients’ homes to provide clinical care

•
Complete administrative tasks and documentation, with flexibility to work from home

•
Participate in team meetings and collaborative care planning

Requirements

•
HCPC registration as an Occupational Therapist

•
Confident in using System One

•
Experience working with neurological patients is desirable

•
Must have a driving licence, access to a car, and business car insurance

•
Ability to work both independently and as part of a multidisciplinary team
