A brief overview of Responsibilities:
Conduct comprehensive psychosocial assessments within 5 days of admission
Assess and address emotional, social, financial, and environmental needs, identifying appropriate interventions
Participate in Interdisciplinary Group (IDG) meetings to develop and revise individualized plans of care
Obtain physician orders in accordance with patient needs and agency protocols
Monitor, document, and communicate changes in patient and family psychosocial status
Maintain complete and accurate clinical records, including assessments, care plans, and progress notes
Evaluate the effectiveness of social work interventions and adjust care plans as needed
Coordinate weekly visit schedules in collaboration with Case Managers
Participate in Quality Assessment and Performance Improvement (QAPI) initiatives
Provide ongoing support to the IDG, even if social work services are initially declined by the patient or family
Assist patients and families with coping strategies and connecting to community resources
Perform additional responsibilities as assigned by agency leadership
Qualifications:
Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
At least one year of Hospice experience is preferred.
The ability to communicate well, both verbally and in writing.
