EPIC Health Partners is seeking a compassionate and motivated Qualified Mental Health Professional (QMHP or QMHP-T) to join our Mental Health Skill-Building Program. The QMHP plays a vital role in promoting recovery, independence, and wellness through therapeutic interventions—all under the supervision of a licensed mental health professional. What You’ll Do
ISP Collaboration: Assist in the creation and ongoing revision of ISPs tailored to the needs of each individual.
Ensure Plan Implementation – Monitor and ensure that ISPs are being effectively carried out.
Case Management – Provide linkage to resources and case management support for individuals.
Timely Documentation – Submit necessary documentation according to established timeframes.
Collaborative Approach – Maintain continuous communication with individuals and key stakeholders (with signed authorization) to ensure coordinated planning and service delivery.
What We’re Looking For
Registered as a QMHP or QMHP-T with the VA Board of Counseling.
Bachelor’s degree or higher in a Human Services-related field (e.g., Psychology, Social Work, Counseling).
At least 1 year of behavioral health experience.
Valid driver’s license and reliable transportation.
Current CPR/First Aid certification and TB screening.
Work Environment
Field-based role requiring travel to client homes and community locations.
Must be able to sit, stand, and engage with clients for extended periods.
Flexible scheduling, including evenings or weekends, may be required.
