Starting Salary Range: $59,000 - $60,864 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change

RADIAS Health is hiring a Mental Health Practitioner to join our Whatever It Takes (WIT) program in providing professional, quality program services to clients who have barriers to successful discharge from state facilities, local community hospitals and IRTS facilities. Work with other members of the team to assist clients in obtaining and maintaining an optimum quality of life consistent with client needs and strengths. Emphasis is placed on working with clients in community settings and directly providing the services necessary to assist clients in achieving their goals. Coordinate and monitor assessments, treatment planning, and follow up for clients. Serve as an advocate and liaison for and on behalf of residents' rights and benefits.

This person must have excellent clinical and problem-solving skills, strong communication skills, and a good understanding of community resources in the metro area.

Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm

The Mental Health Practitioner position is a hybrid model. Some work is done in the community with clients and other work can be done from home.

Duties & Responsibilities

• Work with the larger treatment team to provide services for clients. Support the development, writing, implementation, evaluation and revision of overall treatment goals and plans in collaboration with the client and the treatment team.

• Develop an individualized crisis plan coordinated with community emergency support services.

• Meet with clients to explain WIT services, assess client needs, establish and implement a care plan to address those needs and arrange for resources necessary for community transition.

• Coordinate services with other county or community agencies including but not limited to AMRTC, MSH, Mental Health Case Management, Adult Protection, and community hospitals.

• Coordination of comprehensive assessments, individualized treatment plans, substance abuse treatment, and other tools necessary to assist clients in the community.

• Assist clients with all aspects of psychiatric rehabilitation and all aspects of obtaining and maintaining housing. Will assist and /or teach clients independent living skills such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting. Will assist clients with making and keeping appointments and any other tasks that are required through the client’s recovery process.

• Attend commitment and other court hearings as needed.

• File reports with Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other mandated/required reports when applicable.

• Actively and assertively engage and reach out to clients’ family members/significant others, including landlords, employers and other treatment team members after obtaining client permission.

• Must be able to transport clients in the community.

• Complete all paperwork, reports, and charting within two business days and in an organized manner.

• Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision.

• Cross train in other programs and provide coverage as needed.

• Accurately track client expenses on at least a weekly basis in Wells Fargo system


Requirements:
• Candidate will have a Bachelor's degree in behavioral health sciences plus 2,000 hours work experience providing proving supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).

• No college degree and 4,000 hours supervised work experience providing proving supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
• Must have car, auto insurance and driver's license.
