Perinatal Health Hub Program Manager

Position Summary

The Perinatal Health Hub Program Manager (Category 1) is responsible for the development, implementation, and operational oversight of the Perinatal Health Hub. This position provides strategic leadership during program initiation and ensures compliance with grant requirements, HRSA standards (if applicable), and organizational policies.

The Program Manager will establish infrastructure, develop policies and procedures, coordinate community partnerships, supervise assigned staff, and oversee data collection and grant reporting. This role requires strong project management, maternal health expertise, and the ability to operationalize program goals into measurable outcomes.

 

Primary Responsibilities

Program Development & Start-Up

Lead planning and launch of the Perinatal Health Hub.

Develop implementation timelines, milestones, and performance metrics.

Establish operational workflows, referral pathways, and care coordination processes.

Identify staffing needs and assist with recruitment and onboarding.

Ensure integration with existing medical, behavioral health, dental, and community services.

Policy & Procedure Development

Develop and maintain program-specific policies and procedures.

Ensure alignment with federal grant requirements and HRSA Compliance Manual standards (if applicable).

Establish clinical and non-clinical protocols for perinatal care coordination, case management, and referrals.

Create documentation standards and quality assurance processes.

Develop risk management and safety protocols in collaboration with leadership.

Outreach & Community Engagement

Build and maintain partnerships with hospitals, OB providers, pediatric providers, social services, home visiting programs, WIC, behavioral health agencies, and community-based organizations.

Coordinate outreach strategies targeting underserved and high-risk perinatal populations.

Represent the organization at community coalitions and maternal health collaboratives.

Develop culturally appropriate educational and marketing materials.

Support stakeholder engagement and sustainability planning.

Staff Supervision & Program Oversight

Supervise and evaluate program staff (e.g., care coordinators, community health workers, peer support specialists).

Provide ongoing coaching, training, and performance feedback.

Facilitate team meetings and case review discussions.

Ensure staff productivity, documentation compliance, and service quality.

Promote trauma-informed, culturally responsive, and equity-centered care practices.

Grant Management & Reporting

Monitor grant deliverables, timelines, and budget allocations.

Collect and analyze program data to measure performance outcomes.

Prepare and submit required grant reports in collaboration with finance and leadership.

Ensure compliance with funding requirements and audit readiness.

Participate in evaluation activities and continuous quality improvement initiatives.

Quality Improvement & Data Management

Develop performance dashboards aligned with grant metrics.

Use data to drive program improvements and address care gaps.

Support maternal health quality initiatives (e.g., postpartum visit completion, depression screening, SDOH referral completion).

Coordinate internal and external program evaluations.

 

Essential Tasks

Develop and maintain program implementation plans.

Draft and revise policies and procedures.

Conduct community outreach meetings and partnership development.

Hire, supervise, and evaluate assigned staff.

Prepare quarterly and annual grant reports.

Monitor program budget in collaboration with finance staff.

Track key performance indicators and outcome measures.

Facilitate case conferences and interdisciplinary coordination.

Ensure documentation meets compliance standards.

Present program updates to leadership and/or Board as requested.

 

Qualifications

Education

Bachelor’s degree in public health, nursing, social work, healthcare administration, or related field required.

Master’s degree (MPH, MSW, MHA, MSN, or related field) preferred.

Experience

Minimum 3–5 years of experience in maternal/child health, perinatal care, public health programming, or community-based health initiatives.

Demonstrated experience launching or managing grant-funded programs.

Experience supervising staff preferred.

Experience in an FQHC or safety-net setting strongly preferred.

Knowledge, Skills & Abilities

Strong understanding of perinatal health, maternal morbidity and mortality, and social drivers of health.

Knowledge of grant compliance and reporting requirements.

Experience developing policies and procedures.

Excellent project management and organizational skills.

Ability to analyze and interpret program data.

Strong written and verbal communication skills.

Ability to build collaborative relationships across organizations.

Commitment to health equity and culturally responsive care.

 

Work Environment & Expectations

Combination of office-based work and community outreach.

Occasional evening meetings for community engagement.

Travel within service area required.

Ability to manage multiple priorities in a start-up environment.
