The Referral & Intake LVN Coordinator provides information to potential agency patients and referral sources regarding the agency's total care category of service in compliance with organization policies and procedures and applicable laws and regulations
Schedule:
Wednesday, Thursday, Friday, Saturday & Sunday 8:30am - 5:00pm
Location:
4 days on-site, 1 day remote
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
• Ordering medical supplies and medications (bipap, tube feeding, CADD pumps, comfort meds, etc.)
• Screen and process inquiries for admission to By the Bay Health’s programs of service
• Determine eligibility in consultation with Hospice Medical Director
• Assume responsibility for assessing initial patient/family needs
• Make telephone contact with potential hospice patients to provide information on agency services
• Assist with implementation and coordination of all services required to initiate start of care for patient admission into agency's programs of care
Education/Experience:
• Completion of a Licensed Vocational Nursing Education Program from an accredited
• Minimum of two years of LVN experience preferably with one year experience in an acute care
• Experience in a home health/ hospice setting preferred but not required
Certifications, Licenses, Registrations:
• Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners
Pay Range:
$36.50 - $46.00 hourly
