LVN Licensed Vocational Nurse - PRN Home Health

Prime Home Health is growing and busy! We are seeking a compassionate and skilled LVN working on PRN basis to join our home health team for in-person, in-home health patient visits in Jourdanton, Pleasanton and surrounding areas. As an LVN on our team,  you will play a vital role in providing comprehensive nursing care to adult home health patients.  You will work closely with the interdisciplinary team to ensure the highest quality of care.  This is not a remote position, you must live in the Jourdanton, Pleasanton or surrounding area in order to see patients in person. 

Responsibilities:

Patient Care: Provide direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms. 

Assessments and Documentation: Conduct thorough assessments of patients' condition, document observations, and maintain accurate and up-to-date medical records, including progress notes, medication administration records, and treatment plans.

Education and Support: Educate patients and their families about their condition, treatment options, and end-of-life care, providing emotional support and guidance to help them cope with the challenges they may face.

Collaborative Communication: Maintain open and effective communication with the interdisciplinary team, patients, families, and caregivers, ensuring the coordination of care, addressing concerns. 

Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.

Professional Development: Stay updated with the latest developments in hospice care, attend educational programs, and participate in ongoing professional development activities to enhance knowledge and skills.

Qualifications:

Valid LVN license issued by the State of Texas.

You must live in the Jourdanton, Pleasanton, Poteet or surrounding areas. 

Minimum of one year of experience as an LVN providing in-home health patient visits.

Experience using Kinnser/WellSky EMR, (or others), efficient with apps such as Slack and others. 

Current CPR BLS certification

Strong clinical skills and knowledge of nursing principles and practices.

Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.

Professional, compassionate and sensitivite to the physical and emotional needs of our patients and their families.

Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.

Valid driver's license and reliable transportation for home visits.

Commitment to maintaining confidentiality and upholding ethical standards in patient care.
