Job Overview:
We are seeking a skilled and compassionate Licensed Vocational Nurse to join our healthcare team. As a Licensed Vocational Nurse, you will play a vital role in providing quality care to patients. You will administer nursing care, under the supervision of a registered nurse, for clients in their place of residence. 

Responsibilities:

Under the direction of the RN, assist in identifying the client's physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.

Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care (POC).

Assists clients in the recovery or maintenance of life functions; prevent illness and relapse of illness.

Assure the continuity of care through the delivery of quality patient care.

Provides effective communication to clients, their families, team members, and other health care professionals, and regular participation in Case conferences/Team Meetings.

Completes appropriate documentation in a timely manner to assure compliance with company policy.

Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.

Requirements:

· Valid Licensed Vocational Nurse (LVN) license in the state of Texas

· 1 year of experience as an LVN in a healthcare setting

· Current CPR Certificate

· A Valid driver's license and auto insurance

· Strong knowledge of nursing principles and practices

· Excellent communication and interpersonal skills

· Ability to work effectively in a fast-paced environment

· Attention to detail and strong organizational skills
