Job Description
Country Manor Home Health Care strives to provide superior health care and services by assisting the client to attain his/her maximum potential for wellness and independence. As the LPN Visit Nurse, you will provide direct care under the direction of a Registered Nurse.
This part-time position offers flexible daytime hours with the potential to transition to full-time in the future. This role may also provide the opportunity, though not required, to work at both our Sartell and Brainerd locations.
Qualifications
Graduate of an accredited Practical Nursing program with current Minnesota LPN license.
Current CPR and First Aid certification.
Knowledge of nursing practices and home health care regulations.
Strong clinical judgment, communication, and documentation skills.
Ability to work independently with minimal supervision and collaborate effectively with others.
Ability to observe changes in client condition and respond appropriately.
Professional, compassionate, and respectful when working with clients and families.
Reliable transportation and ability to meet physical job demands.
Commitment to ongoing learning and seeking RN guidance when needed.
Responsibilities
Provide skilled nursing care in the home according to physician orders and care plans.
Administer medications, treatments, specimen collection, and assist with specialized procedures.
Monitor client conditions and promptly communicate changes to the RN Case Manager and physician.
Educate clients, families, and caregivers on care needs and self-care techniques.
Complete accurate, timely charting, documentation, and physician orders.
Provide direction and support to Home Health Aides and Homemakers.
Coordinate care with therapy staff, pharmacies, physicians, and outside agencies.
Participate in staff meetings, in-services, committees, and on-call rotation.
Maintain a safe, supportive environment focused on client well-being.
Perform other duties as assigned.
