As we continue this exciting growth, we are actively expanding our Lake Charles team in preparation for increasing our capacity to 42 beds—allowing us to reach and care for even more individuals in need of specialized behavioral health services.

The Case Manager plays a key and impactful role in guiding patients through their treatment journey. In this position, you will coordinate and oversee patient care, ensuring each individual receives the right services and support every step of the way. From conducting assessments to developing personalized treatment and discharge plans, you will work closely with physicians, nurses, therapists, and community partners to meet each patient’s unique mental health and social needs.
You will be instrumental in ensuring continuity of care by arranging follow-up appointments, connecting patients with housing resources, coordinating therapy services, and linking them to essential community support systems after discharge. As a trusted liaison between patients, families, and the treatment team, you will provide education, advocacy, and guidance—making a meaningful difference in each patient’s recovery journey.

PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:
Education: A psychology major is preferred or nursing education
License: Nursing License, Social Worker License, &/or LPN
Experience: 1+ years coordinating utilization review and discharge planning activities
