Help families navigate the messy parts.. grief, fear, and paperwork.
Lead with compassion, advocate fiercely, and remind people that even in loss there’s still love and dignity.
Collaborate closely with nurses, chaplains, aides, doctors and volunteers in our interdisciplinary team (IDT), ensuring care is heart-centered.
Develop and execute patient-specific care plans
Offer resources, counseling, and that grounding presence people remember long after the goodbye.
What you bring:
A current Social Work License in the state of Georgia.
Hospice, medical, or mental health experience. (and if not hospice yet, a calling to it)
A steady heart that can hold both tears and laughter in the same breath.
Strong communication, organizational, and advocacy skills.
