Position Summary
We are seeking qualified Licensed Practical Nurses (LPNs) to provide direct patient care services under the supervision of Registered Nurses and physicians at the VA Health Care System. LPNs will support geriatric patients in Community Living Centers, ensuring care meets or exceeds VA and national standards.
Essential Duties and Responsibilities
Provide direct nursing care within scope of practice under RN or physician supervision
Administer medications and treatments as ordered
Monitor, document, and report patient vital signs and condition changes
Assist patients with activities of daily living (ADLs)
Support care coordination and interdisciplinary communication
Maintain accurate and timely clinical documentation
Follow VA policies, infection control procedures, and safety standards
Maintain patient dignity, privacy, and confidentiality
Qualifications
Required:
Current, unrestricted Licensed Practical Nurse (LPN) license
Minimum of two (2) years of recent LPN experience
Graduate of an accredited Practical Nursing program
Current Basic Life Support (BLS) certification
Ability to communicate effectively in English
No adverse licensure actions or negative clinical indicators
Preferred:
Geriatric or long-term care experience
Experience in inpatient or institutional healthcare settings
Compliance & Credentialing Requirements
Successful completion of VA background investigation
Completion of VA-required training and annual competencies
Proof of immunizations, TB testing, and annual influenza vaccination
Clearance through HHS OIG Exclusion List review
Schedule & Work Conditions
Multiple shifts available (8-, 10-, or 12-hour shifts)
Facility operates 24/7
No on-call, overtime, or holiday shifts authorized
Schedule provided in advance; subject to patient care needs
