Position Summary
Care Foundation Services is seeking a Contracted Licensed Practical Nurse (LPN) to support time-limited clinical infrastructure projects. This role is project-based for approximately 90 days and will focus on strengthening clinical documentation, care planning, and nurse delegation systems. The LPN will work collaboratively with the Clinical Supervisor and operations team to ensure regulatory compliance, clinical accuracy, and high-quality client care.
Project Phases & Core Responsibilities
Phase 1: Orders & MAR Management
Obtain, verify, and organize medical orders from providers.
Review existing medical documentation for completeness and accuracy.
Update and maintain Medication Administration Records (MARs) in the agency’s Electronic Health Record (EHR) system.
Ensure MARs align with current provider orders.
Identify discrepancies and coordinate corrections with providers and clinical leadership.
Phase 2: Care Planning & Service Documentation
Complete and/or update:
Care Plans
Individual Service Plans (ISPs)
Client-specific clinical protocols
Ensure plans are individualized, person-centered, and aligned with assessed needs.
Integrate medical, behavioral, and support needs into unified care documentation.
Collaborate with case managers, program staff, and clinical leadership as needed.
Phase 3: Clinical Protocol & Nurse Delegation Review
Review existing clinical protocols for accuracy, clarity, and compliance.
Collaborate with the Clinical Supervisor to revise and finalize protocols.
Support nurse delegation processes, including:
Development of delegation instructions
Review of delegated tasks for safety and compliance
Support for documentation related to nurse delegation
Ensure delegated tasks align with state regulations and nursing standards.
General Responsibilities
Maintain compliance with state nursing regulations and agency policies.
Ensure all clinical documentation meets regulatory, payer, and agency standards.
Communicate regularly with the Clinical Supervisor regarding progress and barriers.
Meet project timelines and deliverables within the 90-day period.
Maintain confidentiality and HIPAA compliance at all times.
Qualifications
Required:
Current, unrestricted LPN license in the state of practice
Experience with medication management and MAR documentation
Experience with care planning and service planning in healthcare or human services
Knowledge of nurse delegation principles
Strong documentation and organizational skills
Experience using Electronic Health Record (EHR) system
Preferred:
Experience in developmental disabilities, behavioral health, or long-term care
Familiarity with waiver services or community-based programs
Experience working in project-based or contract roles

Contract Terms
Duration: Approximately 90 days
Hours: Approximately 20 hours per week

Requires use of agency EHR and secure systems
