Join our team and make a positive impact on the lives of others! We are looking for a Licensed Practical Nurse (LPN) for our Colorado team.
This role is field-based position traveling to visit cases in the Greater Denver and Northern Denver markets. There will also be potential quarterly travel to our office in Denver, CO.
Department & Position Overview:
The Licensed Practical Nurse (LPN) will perform clinical evaluations in the home of the Adult Foster Care member to determine the level of service to qualify for the program. The LPN will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are receiving quality service and address any changes in their condition.
Adult Foster Care is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.
Required license:
• LPN – Licensed Practical Nurse; Compact License in approved
Under the direction of the RN, the LPN will travel through out the Greater Denver and Northern Denver area for onsite visits.
The LPN will support consumers/patients virtually in surrounding Denver area by supporting up to 10 virtual/telephonic care plans each day and do Onsite visits for specified region for patients, as identified. No more than 100 site visits completed in a month.
The LPN will support both PreCare and Care divisions.
• PreCare: Development of initial care plan to support the Direct Care Services Calculator provided from the Case Management Agency
• Care: Updated care plans as identified with renewed authorization and/or change in condition. Support supervisory visits as needed, standard of 1 per quarter/patient.
Ideal Candidate Will Possess:
• Licensed Practical Nurse licensed by Colorado Board of Registration in Nursing
• Bachelor's Degree in Healthcare Administration
• Knowledge and experience in developing Care Plans.
• Minimum of 2 years of clinical experience and at least 1 year of experience with elders or adults' with disabilities
• Valid driver’s license with an insured vehicle for travel within the state of Colorado
• Ability to travel in the field to accompany patients to appointments and meet patients in person when needed
• Strong communication and assessment skills; the ability to relate to patients, their families, and community care providers, along with ability to handle rapidly changing crisis situations
• Experience with Salesforce (preferred)
• Ability to manage high volume caseloads
• Able to express empathy and compassion for the underserved
• Strong communication and interpersonal skills.
• Problem-solving and decision-making abilities.
• Excellent communication, critical thinking, and organizational skills
• Experience using a CRM (Salesforce is a plus)
• Able to thrive in a fast-paced start-up environment
Nice-to-Haves:
• Bilingual Spanish strongly preferred

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $25.00 and $35.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
