Position Overview
We are seeking a skilled and patient-focused Long-Term Care Pharmacist to join our professional pharmacy team. The ideal candidate will provide clinical oversight, ensure regulatory compliance, and collaborate closely with healthcare teams to optimize patient outcomes.
Key Responsibilities
Provide clinical pharmacy services tailored to long-term care residents.
Review and verify prescriptions for accuracy and appropriate therapy.
Conduct medication regimen reviews and provide consultations to care teams.
Collaborate with physicians, nurses, and caregivers to promote optimal outcomes.
Ensure compliance with state and federal regulations and LTC pharmacy standards.
Supervise and guide pharmacy staff involved in dispensing, packaging, and labeling.
Maintain accurate records using electronic health record systems.
Participate in meetings and initiatives to enhance resident care.
Perform other duties as assigned by the Pharmacy Manager.
Qualifications
Education: Doctor of Pharmacy (PharmD) from an accredited program.
Experience: Prior experience in a long-term care pharmacy setting strongly preferred.
Licensure: Active pharmacist license in good standing (state-specific).
Strong knowledge of pharmacy regulations and clinical practices.
Proficiency with pharmacy software (Framework or similar).
Excellent attention to detail, communication, and problem-solving skills.
Team-oriented approach and ability to work effectively in a collaborative environment.

Full-time
Monday–Friday 9:00am-5:00pm with occasional weekend or on-call rotation as needed
