Position Summary: POD Health is seeking a Licensed Clinical Social Worker (LCSW) to join our clinical team full-time, on-site in Brooklyn, NY. In this role, you’ll support patients - by assessing mental health needs, developing care plans, and collaborating with a multidisciplinary team to ensure well-rounded support for conditions such as addiction, depression, and anxiety.
Key Responsibilities:
Conduct thorough biopsychosocial assessments and develop personalized care plans.
Focus on treating addiction, depression, and anxiety, primarily in senior populations.
Liaise closely with the medical team to define care goals and manage patient outcomes.
Educate patients about available services and promote active participation in their care.
Maintain timely and accurate documentation in the Electronic Health Record (EHR).
Participate in weekly clinical supervision and interdisciplinary team meetings.
Uphold NASW ethical and professional standards in all areas of practice.
Build therapeutic rapport with patients from diverse backgrounds while maintaining clinical boundaries.
Responsible for outreach to Pod Health clients for enrollment in the Behavioral Health Integration (BHI) program
Conduct outreach and follow-up calls with clients experiencing crisis situation
Follow up on referrals received from the medical team to coordinate behavioral health support and services
Schedule and conduct therapy sessions for clients through Zocdoc
Required Qualifications:
Master’s degree in Social Work from an accredited institution.
Current LCSW license in New York State required.
Experience supporting patients with addiction, depression, and anxiety.
Strong interpersonal, communication, and documentation skills.
Ability to work independently and collaboratively within a team-based model.
Commitment to HIPAA compliance and maintaining confidentiality.
Passion for serving aging and diverse communities.
Preferred Qualifications:
Bilingual in Spanish or Chinese.
Familiarity with telehealth platforms or virtual care settings.
