Position Summary
Freedom Behavioral Hospital is seeking a qualified IOP Program Therapist to join our multidisciplinary team in Cleveland, MS. This role is responsible for delivering high-quality behavioral health services in an outpatient setting, including group, individual, and family therapy, while supporting patients through the continuum of care.
The Program Therapist plays a critical role in treatment planning, case management, and discharge coordination, ensuring patients achieve optimal clinical outcomes and successful reintegration into the community.
Key Responsibilities
Provide group, individual, and family therapy in an Intensive Outpatient Program (IOP) setting
Develop, implement, and update individualized treatment plans
Complete psychosocial assessments and clinical documentation in accordance with regulatory standards
Coordinate discharge planning and connect patients with appropriate community resources
Collaborate with the multidisciplinary treatment team (physicians, nursing, case management, etc.)
Ensure compliance with CMS, Joint Commission (JCAHO), and facility policies
Maintain timely, accurate, and complete documentation in the medical record
Participate in team meetings, case reviews, and performance improvement initiatives
Minimum Qualifications
Master’s degree in Social Work, Counseling, or related behavioral health field
Current licensure in Mississippi as one of the following:
LCSW (Licensed Clinical Social Worker)
LMSW (Licensed Master Social Worker)
LPC (Licensed Professional Counselor)
Preferred Qualifications
Minimum of one (1) year clinical experience in a behavioral health or psychiatric setting
Experience in IOP, inpatient psychiatric, or outpatient mental health programs
Strong skills in group therapy facilitation and crisis intervention
Knowledge of discharge planning and community resource coordination
