QUALIFICATIONS: Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F
SALARY RANGE: As per salary grid
HOURS OF WORK: 35 hours per week

As part of a multidisciplinary team the MHFRT will be the first point of contact for individuals seeking; ongoing mental health service, brief solution focused interventions as well as assistance to address an acute crisis. The Mental Health First Responder will conduct detailed assessments and gather relevant collateral information to determine service needs. Based on the outcome of the assessment individuals will be provided with the most appropriate service. As part of a multidisciplinary team the Mental Health First Responder will also provide acute crisis support to individuals via phone, face to face and mobile intervention.
Reporting to the Integrated Manager of Client Services, the incumbent will be responsible for the following functions:
Complete detailed standardized inter-RAI ESP assessment to determine client needs and services
Providing intervention and brief solution focused support to individuals experiencing a psychiatric crisis (intervention may be via telephone or by personal contact);
Providing a quick response to urgent situations that arise;
attend as needed daily or weekly psychiatry rounds, receive orders from psychiatry and provide follow up, coordinate care with agency staff who have clients on the inpatient unit and document in Cerner when appropriate;
Providing client needs identification through on-going assessment and team oriented treatment planning.
Implementing treatment plans that focus on strengths, wellness and empowerment to support recovery and goal achievement.
Providing crisis prevention and intervention planning with the intention of preventing a relapse and/or hospitalization.
Engaging, educating, and supporting families through the treatment and recovery process;
Medication reconciliation, assessment of medication efficacy, tolerance and side effects including health teaching.
Routinely assesses client’s physical and mental health and liaises with appropriate physicians/psychiatrist regarding identified concerns.
Acting as a resource person or an advocate to assist individuals in obtaining information/accessing services/supports which are deemed needed
