QUALIFICATIONS:
Masters degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse with current registration with the CNO and or a Bachelors Degree in Social Work with a current registration with the O.C.S.W.S.S.W
Lived experience as a parent or primary caregiver is considered an asset
Experience navigating or supporting families in the mental health system
SALARY RANGE: Case Management Grid
HOURS OF WORK: 35 hours per week

The Family Education Clinician position will provide support and education to families. This role is responsible for providing highly individualized and flexible intake, assessment and support to families who are supporting a loved one experiencing a mental health challenges. The Family Education Clinician will also work with the Early Intervention Team, working with families of individuals experiencing a First Episode of Psychosis.
The needs of this population are complex and will require a highly intensive multidisciplinary, integrated team approach. It will be equally important for the Family Education Clinician to be aware of, and be knowledgeable about, the developmental stages of adolescence and young adulthood. The Family Education Clinician will be required to develop, implement and evaluate individualized treatment plans with clients on an ongoing basis.
This position will require The Family Education Clinician to have well-developed group facilitation skills which will be utilized for the purpose of providing groups for clients, families and the community.
Reporting to the Integrated Manager of Client Services, the incumbent will be responsible for the following functions:
Engaging, educating and supporting families through the treatment and recovery process utilizing individual and group intervention strategies;
Conducting detailed assessments;
Compiling relevant demographic and biopsychosocial history information for the purpose of continued case management support or referral/linking to another more appropriate community service;
Providing client needs identification through ongoing assessment and team-oriented treatment planning;
Implementing treatment plans that focus upon strengths, wellness and empowerment to support recovery and goal achievement;
Providing crisis prevention and intervention planning with the intention of preventing a relapse and/or hospitalization;
Family Education aims to teach families about psychosis and its treatment, reduce relapses by encouraging medication adherence and monitoring early warning signs of relapse, support the client’s work towards personal recovery goals, and reduce family stress through improved communication and problem-solving skills.
Provide education regarding mental illness and the role of pharmacology;
Incorporate families into the crisis prevention and intervention planning process with the intention of preventing a relapse and/or hospitalization;
Support family case conferences to review client treatment plan and goals
