Job Description

Intensive Case Manager –  Housing First

Reporting to Deputy Manager

Contract Type - 12 months with strong possibility of permanency

Multiple vacancies

Dublin - Willingness to travel

Fulltime - 35 hours per week (5/7 days)

1. Key Areas

To work directly with service users, utilising a case management approach to ensure access to relevant wraparound services needed for the service user’s
progression.

Advocate for and empower service users to facilitate access to support services

Case Management of assigned service user caseload

Delivering on DRHE Case and Case Management Protocols.

Assessment of new service users and service user induction

Building relationships with partners and identified support agencies

Provision of crisis prevention and intervention strategies.

Referral to health, social, addiction and other specialist resources and community services as required.

Monitoring of support requirements of participants to sustain their accommodation i.e. rent payments etc. and work with the participant utilising motivational and recovery orientated techniques to ensure sustainment of accommodation.

Adherence to Organisational Health & Safety Policies

Strive to work creatively and take initiative to assist the service in achieving its core objectives and ensuring that the needs of service users are met.

·Participation in Out of Hours On Call Support Rota

2. Person Requirements

Must possess a minimum of 2 years of experience in the homeless sector or a related field, with at least one year dedicated to coordinating support plans for service users with complex needs

Have a deep understanding of the principles of Housing First

Have a deep understanding of the root causes of homelessness and the challenges faced by individuals at risk.

Candidates should have experience collaborating with various agencies in both voluntary and statutory sectors

Experience in lone or off-site working is desirable

Clear comprehension of the case management approach is crucial for success in this role.

To attend and participate in team meetings and staff team days.

To attend all mandatory training and any training deemed relevant to service delivery.

To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.

To respect service users’ rights to confidentiality in our relationship with them as per the Depaul Confidentiality policy and code of conduct.

To promote respect, diversity and equal opportunities.

Annual Leave Up to 3 years' service: 25 days per year
3 years' up to 5 years' service: 27 days per year
Over 5 years' service: 30 working days per year (Exclusive of bank and statutory holidays)
10 public holidays each year with Good Friday as discretionary day. Day off and day pay for Christmas Day, St.Stephen's Day, and New Year's Day worked for shift workers.
Non-Shift Workers: Paid day off on public holidays.
Shift Workers: Expected to work on public holidays; receive an additional day of annual leave in lieu.
Non-Rostered Employees: May be entitled to one-fifth of the normal weekly rate of remuneration for the public holiday.
