Intensive Care Navigator
12-month Contract with possibility of extension (New Position)
To empower people in their healing journeys by supporting clients with intellectual disabilities and problematic substance use. The program is funded by Surrey Place through the Safer Steps My Way: Community Outreach for Drug and Alcohol Use that will incorporate the voices of those with lived experience to develop and pilot test a harm reduction program that provides optimal client-focused care for individuals in the Kenora area.
This position provides trauma informed case management services to adults with intellectual disabilities and problematic substance use. This includes leading, developing, facilitating, coordinating, monitoring and updating a person-centered plan with clients, caregivers and service providers that is rooted in Indigenous principles and values.
NOTE: Traditional practices involving burning sacred medicines including tobacco, sweetgrass, sage and cedar and other cultural protocols occur regularly within our work setting.
Qualifications:
Diploma in Social or Health Sciences or related field or a combination of education, training and experience supporting people with intellectual disabilities and/or problematic substance use.
deep understanding and experience of Indigenous culture, values, and perspectives as well as traditional and contemporary healthcare practices, and wholistic health and wellbeing; ability to speak and understand Anishinaabemowin a definite asset;
Extensive knowledge and experience with trauma, addictions, mental health, and homelessness.
Demonstrated knowledge and skills in group facilitation, planning, and case management, and the ability to promote and build consensus and resolution.
Comprehensive theoretical and practical knowledge of the principles of harm reduction, trauma and concurrent disorders, and social determents of health.
Ability to work in a collaborative, de-colonial, trauma-informed and client-centered approach, and to support WNHAC values in meaningful ways;
ability to drive change, motivate, organize and respond to employees with diverse areas of expertise and life experience;
superior organizational and time management skills; committed to consensus-building approaches to problem solving and decision-making; knowledge of and commitment to community-based health and resources;
experience working in an Indigenous organization; understanding and/or willingness to learn of Indigenous culture and values;
proof of immunization in compliance with policy requirements is mandatory;
valid drivers’ license, own vehicle and willingness and ability to travel required;
criminal records check and current drivers’ abstract required;
commitment to ongoing training and professional development relevant to job requirements; and
positive attitude and capacity to act as a healthy lifestyle role model
Salary: $55,771-$72,769 with excellent benefits, including HOOPP pension plan. Salary is based on education and experience.
