The Infection Prevention and Control (IPAC) Hub builds capacity for sustainable implementation of IPAC practices to protect the vulnerable populations in congregate living settings (CLS), including long term care homes, retirement homes, group homes, supportive housing and shelters. William Osler Health System’s IPAC Hub provides tailored guidance, networking opportunities, coaching, mentoring, training and education to settings within the Central West regions of Brampton, West Woodbridge, North Etobicoke, and Malton. As a member of this Hub, the IPAC Practitioner facilitates infection control activities and is considered a change agent with the primary goal of reducing risk of infections. The IPAC practitioner directly reports to the IPAC Hub Manager and is part of the Integrated Health Systems Program.
Accountabilities:
 
• Provide clinical IPAC leadership to congregate living settings within the Central West region
• Oversee and facilitate a local Community of Practice
• Flag infection control concerns to the CLS and IPAC Hub manager
• Support IPAC planning and capacity building
• Share IPAC best practices and clinical direction
• Conduct clinical IPAC assessments
• Provide rapid access to IPAC expertise
• Work in collaboration with regional Public Health and Ontario Health supports
• Report on assessments and risks for identified homes and provide IPAC education appropriate to settings
• A degree in health related discipline preferred (e.g. nursing, laboratory, epidemiology and other allied health)
• Current registration and in good standing with a regulatory body in Ontario
• Previous experience as an infection control practitioner in an acute care setting or long term care home setting is preferred
• Completion of an entry level 1 infection control course required
• Certified in Infection Prevention and Control preferred or commitment to complete within two years required
• Knowledge and/or job related experience in health care practices
• Strong knowledge of infectious diseases, microbiology, epidemiology and statistics
• Proficiency in the use of computer databases, spreadsheets, MediTech and literature search
• Demonstrated ability to work collaboratively in a multidisciplinary team environment
• Self-directed, well-organized and detail-oriented
• Excellent interpersonal, communication and presentation skills

• Excellent attendance, punctuality and work record
• Demonstrated skill in teaching and strong written communication skills
• Able to participate in the IPAC on-call rotation and travel within the Central West region
Hours: Need to be available Monday to Sunday (variable hours), days/evenings (subject to change in accordance with operational requirements) 
Salary:
Minimum: $50.62
Maximum: $62.99
