CHEMICAL HEALTH ASSESSOR - INDEPENDENT CONTRACTOR POSITION
Chemical Health Assessor Position Overview - Fully Remote**
The Chemical Health Assessor is a professional who plays an important role in the interdisciplinary team at Transformation House. They are responsible for providing comprehensive assessments to patients seeking treatment recommendations or admissions.
Time Allocation:
Consulting- minimum of 4 available assessment slots per week/16 per month and a 6 month commitment
Compensation: $60 per assessment/admission
Knowledge, skills and abilities:
Must understand the impact of cultural and individual difference within varied populations
Broad knowledge of ASAM Criteria and making placement recommendations
Excellent communication skills
Strong problem solving skills
Demonstrates collaborative relationships with all members of the Transformation House team to support patient care
Familiarity with healthcare technology and software
Ability to collaboratively work with patients, providers, staff, and partners
Works well independently and as part of a team
Strong written and verbal communication skills
Ability to quickly adjust/respond to an evolving workload
Excellent time management and organizational skills, attention to detail
Responsibilities:
Provide chemical health assessments
Complete timely documentation of assessments provided to meet Minnesota DHS requirements as well asTransformation House policies and procedures
Provide recommendations for appropriate levels of care.
Requirements: To qualify, applicants must meet the following criteria:
Fully licensed as an LADC (no temporary licenses)
Must pass a State of Minnesota Background Check.
This is a remote position, must live in the state of Minnesota
Preferred:
Experience working with substance use and co-occurring disorders
