The Hospice Social Worker establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organization’s mission and philosophy.
EDUCATIONAL REQUIREMENTS
Master’s degree in Social Work from an accredited program
Licensure by Georgia Board of Professional Counselors, Social Workers and Marriage & Family Therapists
EXPERIENCE REQUIREMENTS
Minimum of one (1) year experience as a Social Worker in a healthcare setting
Knowledge of the Medicare Certified Hospice Program preferred.
OTHER REQUIREMENTS
Possess a valid driver’s license in the state of Georgia.
Ability to either drive or have available reliable transportation and present evidence of valid automobile liability insurance.
Ability to communicate effectively, orally and in writing, and to comprehend written and verbal communication.
Ability to operate computer equipment, fax, and copier machines.
Ability to work a flexible schedule.
Ability to work well within an interdisciplinary team setting as well as independently.
Ability to work with patients and families dealing with life-threatening illness and death.
Ability to assess and respond to the needs of patients and families in varied settings.
Ability to make meaningful observations and write comprehensive accurate reports.
Demonstrates excellence in managing human resources.
Demonstrates effective public speaking skills and the ability to develop good interpersonal relationships.
