You will be responsible for the hospice case management of your assigned patients on an intermittent basis in their place of residence. Brief Overview of Responsibilities for Registered Nurses:
The Hospice RN is responsible for completing an initial assessment of patients and their families to determine hospice care needs and developing individualized care plans focused on comfort and quality of life. This role coordinates care with the interdisciplinary team while supervising LPNs and hospice aides involved in patient care. The RN provides direct patient support, including pain and symptom management, and regularly monitors, documents, and reports changes in the patient’s condition. Responsibilities also include obtaining physician orders, adjusting and re-evaluating the plan of care as needed, participating in on-call duties according to policy, and performing other duties as assigned to support patient care.
Qualification of Registered Nurses:
Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience is required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.
