We are a locally owned, mission-driven hospice serving patients and families throughout the Salt Lake City and surrounding areas with compassion, dignity, and clinical excellence. We are currently seeking a full-time Hospice Case Manager (RN) to join our growing interdisciplinary team.

Key Responsibilities
Manage and coordinate hospice care for an assigned patient caseload
Perform comprehensive assessments and develop individualized plans of care
Collaborate closely with physicians, social workers, aides, chaplains, and other team members
Provide symptom management, medication oversight, and patient/family education
Ensure accurate, timely clinical documentation and regulatory compliance
Participate in interdisciplinary team meetings and on-call rotation as scheduled
Qualifications
Current Utah RN license in good standing
Hospice or home health experience strongly preferred
Strong clinical judgment, organization, and communication skills
Ability to work independently while functioning as part of a collaborative team
Compassionate, patient-centered approach to care
Reliable transportation and willingness to travel to patient homes
