This person will support the Central and Southern area of Austin, and as such, must live in those areas.
A Day in the Life:
Start your day visiting patients in their homes, focusing on comfort, dignity, and quality of life
Assess patient needs, manage symptoms, and adjust plans of care as conditions change
Coordinate closely with physicians, hospice aides, LPNs, social workers, and chaplains
Support and educate families, helping them feel confident and cared for
Document care clearly and efficiently in our EMR, Consolo.
Advocate for your patients while maintaining autonomy in how you structure your day
What You’ll Be Doing:
Serve as the primary nurse coordinating care for your assigned patients
Develop, implement, and update individualized plans of care
Determine visit frequency based on patient acuity and family needs
Provide guidance to Hospice Aides and LPNs to ensure excellent care
Communicate changes in condition to physicians and the care team
Participate in on-call rotation
Background You’ll Need:
Active RN license in the state you’ll practice
Graduate of an accredited nursing program (ADN or BSN)
2+ years of RN experience in the hospice setting, working for a hospice agency/program is highly preferred, but not required.
Strong clinical judgment, compassion, and communication skills
Reliable transportation, valid driver’s license, auto insurance, and CPR certification
