Position Overview
The Hospice RN Case Manager provides coordinated, patient‑centered care to individuals and families facing life‑limiting illness throughout the San Antonio area. This role serves as the primary clinical resource for assigned patients, ensuring comfort, dignity, and care aligned with hospice philosophy and patient goals.
Essential Responsibilities
Conduct comprehensive patient assessments and develop individualized plans of care
Serve as the primary clinical liaison for patients, families, physicians, and hospice team members
Provide skilled nursing visits, symptom management, and compassionate end‑of‑life care
Educate patients and families on disease progression, comfort measures, and available hospice services
Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members
Monitor patient condition and adjust care plans as needed
Advocate for patient preferences, goals, and dignity at all stages of care
Maintain timely, accurate clinical documentation in compliance with hospice regulations
Qualifications
Current Registered Nurse (RN) license in the State of Texas, in good standing
Hospice experience preferred (home health or case management experience welcomed)
Strong clinical assessment and critical‑thinking skills
Excellent communication skills with patients, families, and care teams
Ability to work independently while remaining closely connected to a team
Bilingual (English/Spanish) a plus, reflecting the diverse San Antonio community
