Hospice Medical Social Worker (MSW)
Dallas–Fort Worth, TX (Fort Worth / Arlington / Dallas Territory)

• Travel: Local field travel within assigned DFW territory
• Salary Range: $72,000 – $85,000 annually (based on experience and licensure)
Overview
We are seeking an experienced Hospice Medical Social Worker (MSW) to support patients and families throughout the Dallas–Fort Worth metroplex. This field-based role is a core member of the hospice interdisciplinary team (IDT) and is responsible for delivering comprehensive psychosocial support in accordance with Medicare Conditions of Participation (CoPs) and Texas hospice regulations.
This is an ideal opportunity for a hospice-experienced social worker who values autonomy, compassionate end-of-life care, and collaborative team practice.
Responsibilities
• Conduct comprehensive psychosocial assessments for hospice patients and families.
• Develop, implement, and revise individualized social work care plans.
• Provide counseling related to terminal illness, anticipatory grief, caregiver stress, and end-of-life adjustment.
• Assist with advance care planning and goals-of-care discussions.
• Coordinate community resources and facilitate care transitions.
• Participate actively in interdisciplinary team (IDT) meetings.
• Maintain timely, accurate, and compliant documentation within the EMR.
• Support regulatory compliance with Medicare hospice CoPs and Texas licensure standards.
• Provide bereavement and caregiver support as appropriate.
Required
• Master’s Degree in Social Work (MSW).
• Active Texas LMSW or LCSW license in good standing.
• Minimum of 1 year hospice experience.
• Valid Texas driver’s license and reliable transportation.
• Ability to work independently in a field-based environment.
Preferred
• Home-based hospice experience.
• Strong understanding of hospice IDT model and Medicare regulations.
• Experience with end-of-life counseling and advance care planning.
