Gallagher Hospice is Hiring an Admissions RN with Hospice Experience

($10,000 Sign-On Bonus)

We invite you to apply to join our growing hospice team at Gallagher Hospice, where you’ll make a meaningful difference in the lives of individuals and their families. Position hours are 10:00 am to 6:30 pm

Key Responsibilities:

Comprehensive Patient Assessment: Perform initial, ongoing, and detailed assessments to determine hospice needs, ensuring that each patient’s care plan is personalized and effective.

Develop and Reassess Care Plans: Collaborate with patients and families to develop and regularly update care plans that reflect changing needs and preferences.
