Outsourcing Advantage is actively seeking a compassionate and motivated on call work from home Agent for our Homecare Agency team. In this role, you will be instrumental in coordinating home healthcare services for clients, ensuring they receive high-quality care tailored to their specific needs. You will engage with clients, families, and healthcare providers to facilitate care plans and manage service delivery effectively.
Responsibilities
• Match Home Health Aide (HHAs) & Nurses to available shifts.
• Coordinate scheduling of caregivers and adjust assignments as needed to ensure continuity of care.
• Maintain ongoing communication with clients, families, by responding to emergency care requests from families or clients and care staff.
• Make outgoing attendance calls to the caregivers who have a scheduled shift but haven't clocked in their attendance
• Coordinate caregiver assignments and maintain accurate scheduling
• Collaborate with nurses, case managers, and other team members to support patient services
• Track patient services and ensure documentation is complete and compliant
• Respond to patient inquiries with empathy and professionalism
• Complete daily/weekly task reports
• Staff open cases daily or as needed
Requirements:
• Excellent verbal and written communication skills in English & Spanish at least at a level of C1
• Excellent organizational and multitasking abilities, with high attention to detail.
• Previous experience in home care, healthcare coordination, or a related administrative role
• Familiarity with HHA exchange or scheduling software.
• Empathetic and compassionate attitude toward clients and their families.
• Ability to work both independently and collaboratively within a diverse team.
• Proficiency in Microsoft Office Suite and client management software.
