Home Health Registered Nurse (RN)

We are specifically looking for RNs with a background in home health care experience. Applicants must have prior experience in home health nursing to be considered for this role.

Our agency is located in Agoura Hills, CA, we provide services throughout Los Angeles County and extending into the Mid/Central Valley.

Duties:

Make an initial health evaluation and individualized plan of care

Administer medications and assist with pain management

Clean and dress wounds

Document symptoms and vital signs

Monitor patient health and update care plan accordingly

Instruct patients and their families on proper home care

Provide suggestions to improve safety at home

Detect early symptoms that could lead to a hospital visit

Supervise home health aides

Communicate with physicians, social workers or other health advisors

Provide encouragement and support

Skills:

- Current Registered Nurse (RN) license in the state of practice
- Proficiency in using electronic health record systems (e.g., DataSoftLogic)
- Familiarity with durable medical equipment (DME) and its use in patient care
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
