We are currently seeking a dependable and compassionate Licensed Nurse (LPN or RN) to provide twice-daily skilled nursing visits for a client in Owensville, Ohio.

Position Overview

This role involves providing morning and evening skilled nursing visits in the client’s home. Ideal for nurses seeking consistent, structured visits with minimal travel.

Responsibilities

Provide skilled nursing care per physician orders and plan of care

Medication administration and management

Health assessments and vital signs monitoring

Documentation in accordance with OASIS and agency standards

Communication with physicians, case managers, and family members

Ensure client safety and quality of care

Schedule

Twice daily visits (AM & PM)

Flexible timing within scheduled windows

Consistent ongoing case

Qualifications

Active Ohio LPN or RN license (required)

Home health experience preferred

Current CPR certification

Reliable transportation and valid driver’s license

Ability to chart accurately and timely

Strong clinical judgment and communication skills
