We are currently seeking an experienced LVN Home Health Clinical Coordinator to join our leadership team and support the ongoing success of our clinical operations.
Position Overview:
The LVN Home Health Clinical Coordinator oversees the coordination and supervision of daily clinical activities to ensure timely, high-quality care. This position works closely with the Director of Nursing, field clinicians, and administrative staff to uphold regulatory compliance, improve clinical outcomes, and enhance the patient experience.
Responsibilities:
Coordinate and oversee patient care delivery, ensuring compliance with physician orders, agency protocols, and regulatory standards
Manage and monitor staff assignments, patient schedules, and case load distribution
Review and approve clinical documentation
Facilitate case conferences, patient status reviews, and interdisciplinary collaboration
Provide clinical guidance, training, and support to field staff
Assist with admissions, recertifications, and discharges as needed
Ensure timely submission of documentation to support billing and regulatory deadlines
Participate in quality assurance, performance improvement (QAPI), and survey readiness
Maintain up-to-date knowledge of CMS Conditions of Participation and home health regulations
Qualifications:
Active LVN license in the state of Texas (required)
Minimum 2 years of clinical experience in home health (required)
Prior supervisory or case coordination experience (preferred)
Proficiency with EMR systems (Kinnser/WellSky preferred)
Strong understanding of Medicare regulations
Excellent leadership, communication, and problem-solving skills
Ability to prioritize and multitask in a dynamic environment
