Community Behavioral Nursing Services is a family-owned and operated, accredited home health agency dedicated to empowering individuals with intellectual and developmental disabilities to live with dignity and respect. Unlike traditional home care roles, our nurses typically travel no more than fifteen miles between patient locations, allowing for a better work-life balance, and less time on the road. CBNS proudly serves the Stark, Medina, Cuyahoga, and Lorain Counties- and we are continuing to grow! We are currently seeking a dependable, self-motivated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team in Firestone Park, Ohio!
Compensation: $2,413.40- $2,847.90 bi-weekly

Schedule:
Monday & Wednesday: 5:30 AM–10:25 AM and 3:00 PM–9:40 PM
Every other Friday: 4:20 PM–9:15 PM
Every other Saturday & Sunday: 5:30 AM–10:25 AM and 4:20 PM–9:15 PM
Job Description:
The LPN/ RN provides skilled nursing services to patients in accordance with the current nursing plan of care within their scope of practice. Employees are also responsible for the completion and documentation of vital signs and observations, coordination of care activities, and delivering nursing services for each assigned patient. The LPN/RN also promotes safety and education, working in a professional and organized manner.
Essential Job Functions and Responsibilities:
Provide skilled nursing care to assigned patients, as identified in current nursing plan of care and within nursing scope of practice
Complete and document observations, vital signs, body systems, pain, etc.
Complete skilled nursing procedures, such as specimen collection and related patient services
Promote health and prevention to decrease incidents of hospitalizations and/or re-hospitalizations by providing patient and caregiver education
Act as a professional nursing liaison between patients, caregivers, physicians, and other health care providers
Coordinates care with CBNS staff, other agencies, and health professionals, as needed
Complete medication related tasks, documentation, physician order coordination, medication change-over, medication check-in, and other related tasks
Contribute to a safe and secure environment for patients, caregivers, and other nurses by taking prompt action to remediate; if unable, then escalate to supervisor for instruction and/or assistance
Assist in keeping nursing supplies and equipment clean and up to date at the work site and/or patient’s home
Review and implement nursing plans of care during each patient skilled nursing service, within scope of practice
Participate in CBNS Supervisory Visit and review with the Clinical Manager, at intervals required by regulations and CBNS policy and procedures
Complete incident and infection report promptly, including notification to internal/external contacts, as requested
Electronically verifies location of start and end of each skilled nursing visit by patient in electronic verification record, promptly reporting errors and/or omissions for correction
Complete shift report via secure email system, including copy to Clinical Manager, and others as requested
Uphold and promote the mission of CBNS and the value of home health nursing services
Demonstrate constructive problem solving and team-oriented resolution to issues and/or concerns
Remain in compliance with CBNS policy/procedures, Ohio Board of Nursing scope of practice, and CBNS Code of Conduct, as well as ethical business practices
Qualifications:
Active, unencumbered Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensure in the State of Ohio
Minimum of one year’s experience as a Licensed Practical Nurse (LPN) or Registered Nurse (RN), preferably in a clinical home care setting
CPR Certification
Valid Driver’s License and Auto Insurance
