Home Care Liaison
Personal Care Services Agency | Las Vegas, NV | Full-Time
Reports To: Agency Owner
Location: Las Vegas, NV (Field-based — Clark County territory)
Schedule: Monday–Friday, flexible hours; some evenings/events as needed
Compensation: Base salary + performance bonus (referral-based incentives)
Start Date: ASAP — immediate need
About the Role
We are a growing Personal Care Agency (PCA) serving Las Vegas, Henderson, Summerlin, and North Las Vegas. We are looking for a driven, people-first Home Care Liaison to be the face of our agency in the field.
This is a relationship-building role. Your primary job is to connect with hospital discharge planners, social workers, rehab centers, physicians, senior living communities, and other healthcare partners who regularly refer patients in need of in-home personal care services.

Key Responsibilities
Referral Development
Visit hospitals, SNFs, rehab centers, physician offices, and senior communities across Clark County on a consistent weekly schedule
Build and maintain strong relationships with discharge planners, social workers, case managers, and care coordinators
Present agency services, leave materials, and follow up regularly to drive referral volume
Attend local networking events, senior expos, and community health fairs to raise brand visibility
Account Management
Maintain a contact database (CRM) of all referral partners and track outreach cadence
Communicate client placement updates back to referring partners to build trust and repeat referrals
Identify new referral source opportunities and bring them to the agency's attention
Collaboration & Reporting
Work closely with the intake coordinator to ensure smooth handoffs from referral to enrollment
Provide weekly referral activity reports to ownership
Represent the agency's mission, values, and service quality at all times
Qualifications
Required
2+ years of outside sales, community outreach, or healthcare liaison experience
Strong knowledge of the Las Vegas healthcare landscape (hospitals, SNFs, senior communities)
Excellent verbal communication and relationship-building skills
Self-motivated with ability to manage your own territory and schedule
Valid Nevada driver's license and reliable transportation
Proficiency with CRM tools, Google Workspace, or similar platforms
Preferred
Background in home care, hospice, hospital, or senior care industry
Existing relationships with Las Vegas area discharge planners or case managers
Bilingual (English/Spanish)
Experience working with Medicaid waiver programs or ADSD referral systems
