Title: HCV Registered Nurse

Reports to: Manager, Street Health Centre

Position Type: Full-Time, Contract (35 hours/week, 1 year)

Salary: $66,757.60 - $78,533.00 Annually

Vacancy Status: Existing Vacancy

Location: 115 Barrack St, Street Health Centre

 

Position Summary

This Registered Nurse position will provide care to marginalized community members living with/at risk of acquiring hepatitis C and HIV at various locations throughout the region. This position will provide direct Hepatitis C testing, treatment, case management support, education, and requires ongoing outreach to support individuals living throughout the region. Care is provided in collaboration with an interdisciplinary team, including physicians, nurse practitioners, a Hepatitis C Liaison Worker and Outreach workers, Peers as well as partner agencies. The RN works primarily with people who use substances, have a link to incarceration and individuals experiencing homelessness and ongoing outreach, harder to engage populations such as those living in encampments.

Key Responsibilities 

Clinical

Hepatitis C, treatment, - Engage clients in Hepatitis C testing and treatment, providing health education and referral to support services.

Provide primary health care services within the scope of the RN professional standards including: physical assessment, screening, triage, health education/counselling, vaccinations, venipuncture history taking, managing lab requisitions, referral and follow-up, and other clinical activities according to the College of Nurses of Ontario Standards of Nursing Practice. Consult with medical staff and other providers when the client’s condition requires care beyond the scope of practice (RN).

Ensure clients receive appropriate comprehensive care and treatment by accurately assessing clients and connecting them with needed support and services.

 

Outreach

Support testing and health teaching at multiple sites across the south eastern region.

Engage with their health and social service providers offering support for care and testing of their patients.

Utilize OTN to offer virtual appointments and care for clients outside of Kingston

Working with the HCV Liaison Worker and Outreach Worker to engage and build new relationships with community partners for Hepatitis C testing and education.

Participate in planning and delivering Hepatitis C pop up testing and care clinics in the region.

 

Collaborative Care

Consult with medical staff and other providers when the client’s condition requires care beyond scope of practice (RN)

Engage and build new relationships with community partners and other CHC’s within our region to maintain a regular outreach schedule to provide Hepatitis C testing and treatment.

Registration of clients.

Participate in monthly Hep C team meetings

Support contacting and booking clients for Ultrasound clinics and hepatology clinic

Attend team huddle meetings

Participate in QI efforts

Attend monthly SHC team meetings

Participate in working groups and committees of KCHC as interested.

Supports KCHC’s student and volunteer placement programs

On occasion, perform other temporary duties as required

 

Basic Education and Experience Requirements

Current registration as a Registered Nurse, in good standing, with the College of Nurses of Ontario with a minimum of three years’ experience in direct patient care.

Thorough knowledge of and experience with concerns relevant to clients with Hepatitis C and HIV (such as, but not limited to harm reduction, addictions and mental health);

 

Knowledge, Skills and Abilities

Knowledge of Hepatitis C, harm reduction programs, health promotion initiatives and social determinants of health perspective.

Demonstrated expertise in medical laboratory procedures; thorough knowledge and understanding of infection prevention

Demonstrated knowledge and understanding of infectious diseases generally, and Hepatitis in particular.

Experience and comfort with venipuncture

Knowledge and experience in mental health, harm reduction, addictions and Smoking Cessation program.

High level of comfort with diverse populations.

Proficiency in the use of computers and various applications

Able to work independently and as a team member.

Excellent interpersonal skills and commitment to working in a team.

Excellent organizational skills and attention to detail

 

Other Requirements

Valid Driver’s License, Driver’s Abstract and proof of vehicle liability insurance as this is a requirement under KCHC’s insurance provider (HIROC) 

Access to a vehicle as this position has daily travel to other locations

Current and satisfactory Criminal and Vulnerable Persons Check

Current First Aid and CPR an asset

Ability to meet the physical demands of the position
