Job Summary

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate medical services and support throughout their healthcare journey. Duties

Qualifications

Current Registered Nurse (RN) license in good standing.

Experience in hospice care is preferred.

Previous experience in case management is highly desirable.

Strong understanding of medical terminology and healthcare systems.

Excellent communication skills, both verbal and written.

Ability to work collaboratively within a multidisciplinary team environment.

Strong organizational skills with the ability to manage multiple priorities effectively.
