Ihre Aufgaben:
• Hausärztliche Versorgung: leitliniengerecht, empathisch, mit Fokus auf Chroniker‑ und Präventionsprogramme
• Praxis mitgestalten: Prozesse standardisieren, digitale Tools (z. B. Doku/Transkription) in den Alltag bringen, Kennzahlen mitdenken, Mitarbeit an Praxisprojekten z.B. Tele-Sprechstunde
• Qualität sichern: strukturierte Anamnesen, Medikations‑/Risikomanagement und DMP/Früherkennung
• Teamarbeit: enge Zusammenarbeit mit MFA/VERAH/NäPa, Austausch im Vantis‑Verbund

Das bringen Sie mit:
• Qualifikation: Approbation und den Facharzt Allgemeinmedizin oder Innere Medizin (hausärztliche Versorgung) – oder WBA im fortgeschrittenen WB‑Abschnitt
• Arbeitsweise: Digitalaffinität und Lust den Arbeitsalltag mit innovativen Technologien und klaren Prozessen effizienter zu gestalten, sowie Freude daran, Praxisabläufe aktiv zu entwickeln
• Sprachkenntnisse: Deutsch C1/C2, souveräne Kommunikation mit Patient:innen und Team
